anuary 24 2015.

Campbell Pool, a skydiver with 340 jumps was on a 3-way wingsuit jump. At deployment time, the bridle connecting his pilot chute to his main canopy wrapped around the pilot chute. He cut away the main caopy, and launched his reserve. Once his reserve parachute had successfully inflated, the main canopy fell away, but was still attached by the RSL (https://en.wikipedia.org/wiki/Reserve_static_line). The container he was using has a RSL design that is not used by any other skydiving container manufacturer, and a combination of the RSL design and unfamiliarity with the manufacturers procedures (which are contrary to all other containers) caused his reserve parachute to malfunction. what followed was a terrifying out of control ride to the ground where he crash landed and was taken to hospital. He was incredibnly lucky to escape with only minor injuries form a crash that many observers including staff at the dropzone expected to be fatal.
